An idea for ya or at least what I'd like to do on my 50th...
Before the trip, stow away a box of 50 pictures/keepsakes from hour husband's life...anything from age 0 to 49, doesn't have to be one from every year, just the good stuff - images of happy times/significant moments or small keepsakes like a ticket stub or award that represents those moments. Hide them in a small box that'll fit in a backpack and I'd put them in a ziplock bag too.
Then pick a semi-secluded place on the island - tip of Rams Head, top of Peace Hill, your favorite beach or get adventurous and have a charter drop you off on Sandy Spit or some other really remote place.
Bring along some beers/champagne/cupcakes/candles whatever and just sit there going through all the images and remembering the good stuff.
Then when your hubby is all hopped up on beer, tears, and nostalgia, whip out the video camera and recreate the scene from City Slickers where you ask him what his best day and his worst day was and what his hopes, dreams, and goals are for the next 50 years...or just let him ramble for a while.
Oh, and bonus if you can get him to wear Ruth's Pirate Wench outfit the whole time your doing this...have fun and don't forget your safety word.

Had one other idea for you...have you ever tried video calling on Skype.com?
If you have a laptop with a camera, you could bring it along and take it to a bar or restaurant on STJ that has WiFi. Then you ask your friends and family back home to meet at a bar or wherever and gather around a laptop on their end to sing him Happy Birthday and share some laughs.
Might be a logistical nightmare, but we're bringing our laptop to STJ when we go in June (villa has WiFi) and will be using Skype to call our young son who will be staying with our parents that week...the times I've used it, it's been really easy though and the images are really clear. Plus, you wouldn't believe how much more effective it is when you can see someone on the other end versus just hearing them.
In five years I'm sure this will be very common, but right now it's still pretty new and would be pretty neat if your husband hasn't seen it yet.
