In summary: One form per family. Like others, we keep extra forms on hand but the forms are easy and quick to fill out on the spot if you don't do it in advance. Just estimate the amount of stuff you have purchased on the back of the form. They are just looking for the expensive stuff that you have to pay duty on. It's good to declare something even if its little, otherwise the agents will wonder what you are hiding. Don't sweat it.

With your family of 4 you can have 6000.00 before you have to pay duty so go uot and have a good time

We didnt find a need to pick them up ahead of time. There is someone passing them out whenever they are required. Just have a pen with you because they dont supply those. I filled out our form (one per family) while waiting in line and had plenty of time. No one ever asked me for a receipt for anything.
